Hywel Dda University Health Board has reached an interim agreement with the Trustees of Shalom House.
The nurse-led branch surgery at Shalom House will remain open for another three months, until 31 March 2026.
We are very grateful to the Trustees of Shalom House for their flexibility in agreeing to this extension.

Private Fees & Charges
The NHS does not pay for some of the services we provide. Please note that the Practice can charge for non-NHS Services.
These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions and some Travel vaccinations. We do not sign passport forms.
We require payment of our fee by BACs before we can process any requests/forms.
Please contact the surgery to obtain a copy of our charges.
