Following the announcement by Shalom House that the charity will close its business on the 31 October 2025, Hywel Dda University Health Board (UHB) is in active discussions with the charity about the future of the branch surgery to Meddygfa Penrhyn, which operates from Shalom House on Nun Street in St Davids.

You can exercise your information rights or make a request to access any personal information we hold about you at any time. Requests to access your information must be made in writing to the Practice Manager. You should provide enough information to identify yourself, and in regard to medical records, the period of time or specific treatment you want records for.
